Transition from Corrections
Known as the Transition Program, people are supported through the process of their release to the community from an adult correctional facility.
About the program
The program is a recovery focused support service for people with a mental illness released to the community from an adult correctional facility. Engagement with the client commences in the correctional facility prior to release and support can continue for up to 12 months post release. Support provided includes emotional and practical support.
Eligibility
To be eligible for this service, you need to be:
An adult
(aged 18 years and over)
Referred by the Prison Mental Health Service
Access
This service is accessed through Mental Health service referral while in a correctional facility.

During your support we will assist you with:
On the day release
with transport and support settling back into the community
Supports
to find and maintain a place to live
Attending
mental health appointments
Connecting
you with health and welfare services and local community supports
Accessing
financial support, educational and vocational opportunities and other services
Personal Goals
visioning your future and working towards your goals
The NDIS
assistance to access the NDIS where eligible
