About the program
The service supports people to return to the community after a hospital stay or avoid hospital admission by increasing resilience through clinical, non-clinical psychosocial and peer recovery supports, tailored to support recovery goals.
Participation in the TRS includes:
- intensive one-on-one and group support, with staff on-site 24/7
- support to develop and work through a recovery plan
- a stay in one of the transitional recovery houses for 7-14 days (a shared living environment)
- counselling and crisis support planning
- access to a range of groups, as well as individual community outreach support offered through the Capstone program, after leaving the TRS house
- collaborative safety assessment and planning to support the provision of services, both in the house and in the community
