Transition from Corrections (TFC) supports people with mental illness who are being released to the community from an adult correctional facility. RFQ’s support begins in the prison environment prior to release and continues for up to twelve months post-release.
On the day the person is released, RFQ staff will assist with transportation to the person’s new home. In addition to securing suitable housing for the person post-release, RFQ will provide support around:
