

Transitional Recovery Service (TRS) is a short-term shared living house in the community which supports people to return to the community after a hospital stay, or to avoid a possible hospital admission. Clients generally stay at the house for 7-14 days, during which time they have 24/7 access to peer workers and clinicians on-site.

After clients leave the house, they are provided access to a range of social and support groups offered through the Capstone Program. Capstone works collaboratively to plan transition back home and support individual recovery needs. It further assists clients to increase resilience in the community, along with recovery goal setting and support planning. In addition, the program connects clients to community health services and networks.
