At Wellbeing Connect, we provide a range of tailored, integrated services designed to support each individual’s unique mental health journey. Our dedicated team is committed to empowering people with complex mental health needs to achieve greater wellbeing and lead more fulfilling lives.

Wellbeing Connect supports the Jimboomba community and welcomes referrals from nearby areas.
We work alongside individuals facing significant and complex mental health difficulties, as well as their families and carers, by providing tailored, recovery-oriented support. Our approach is holistic, focusing on the whole person and acknowledging that physical health, emotional wellbeing, and social connection all play an important role in supporting recovery and overall wellbeing.

Our services are available to people experiencing severe and complex mental health challenges who live in Jimboomba or nearby communities.
We work with people whose needs require tailored, recovery-oriented support, ensuring services are delivered in a way that is person-centred, holistic, and responsive to their individual circumstances.
Please see the referral pathways below for more information on how to access our services. Contact us if you have any questions or would like further information.

The process is straightforward and streamlined, with intake, assessment and triage steps to determine to achieve your goals. From there, treatment options, advice, planning and mutual agreements can be made. The goal is to provide support throughout the whole journey to enable a smooth transition and the best outcomes for all relevant parties.

Our Referral & Intake process is designed to make it easy for people to connect with the right support. We work with individuals, referrers and other services to understand the person’s needs, circumstances and goals, and determine how our service may best support them. This includes discussing available supports, providing information about our services and, where appropriate, connecting people with other services that may better meet their needs.
We offer a range of welcoming groups and activities at our Jimboomba centre, including Art Group, Walking Group, Carers’ Group and Cooking Group, as well as a selection of therapeutic groups. These groups provide opportunities to connect with others, build skills, support wellbeing and enjoy shared activities in a supportive environment. Please reach out to us for more information about the groups available and how to get involved.
Individual Care Planning involves working collaboratively with each person to understand their needs, strengths, goals and preferences. Together, we develop a personalised care plan that identifies meaningful recovery goals and the supports and services needed to work towards them. The plan is regularly reviewed and adjusted to reflect changes in the person’s circumstances and priorities.
Psychosocial Support focuses on helping people strengthen their wellbeing, independence and ability to participate in everyday life. Support may include building confidence and coping skills, developing social connections, improving daily living skills, accessing community activities, and working towards personal goals. Support is tailored to each person’s needs and circumstances.
Clinical Care Coordination involves working closely with the person, their support network and other health and community services to ensure their care is well coordinated and responsive to their needs. This may include facilitating communication between services, supporting access to appropriate clinical care, monitoring progress and helping to ensure continuity of care across different stages of a person’s recovery journey.
Recovery-Focused Support recognises that recovery is a personal journey and looks beyond symptoms to focus on what matters most to each individual. We work alongside people to build on their strengths, develop skills and confidence, strengthen connections and work towards goals that support a meaningful and fulfilling life.
